I've never had this before but for whatever reason the nurse at the counter seems to have moved slightly further back than she usually is meaning I can't talk to her to heal. I can interact with her fine before activating the quests and in the same quests in multi-mode but she changes position in one person mode.
I tested other quests and it only seems to be these 2 quests affected. It happens in at least normal and hard but I can't check very hard or ultimate as I don't have access to them yet.
I'm not using any additional addons or custom textures, if that makes a difference.
