I recently started playing some EP 4 missions, and there are areas where everything has an orange filter (heat, I guess?). How does the passive damage I take in those areas work?

Yeah, this is a poison or heat object. It's an object in the quest that defines a circle where a player inside that range will take damage every second after a short period (150 frames it seems), and sometimes there is a fog filter (the orange filter in that case). There's always a sound effect associated with it too that sounds like wind blowing.

The quest object has a specific 'poison power' and the game scales this by difficulty. 1x, 2x, 3x, and 6x for N, H, VH, and Ult. You can't really avoid it other than stepping out of its range.

However, the object is just a radius and those rooms in Episode 4 are irregular shapes, so Sega often placed a handful of them to cover all of a room, and they can overlap. This is especially noticeable in New Mop-up Operation #3 in the first heat room. There are some gaps in the coverage in War of Limits #3 when standing on the rocks, which seems like a feature of the object actually (but Sega placed new Poison objects on the rocks in NMU3, so the heat damage on those is likely not stacked but I don't feel like verifying right now).
