Unable to claim Book of Katana 3 on Hardcore

When trying to turn in the Yakasani Magatama for Book of Katana 3, Claire rejects the item, saying it's not correct.
Forsaken was having issues and I tried to complete it for them and still had issues.
Last week I created a Book of Katana 3 on normal with no issue in normal.

From Forsaken:
"well i tried on all difficulties except Hard on multi-mode
and i tried on ultimate on solo mode, so it's not 'difficulty' locked in case that's something that comes up in the discussion
i also tried submitting items out of order but that didn't work either"

Was this with the same Yasakani Magatama? What about trying with a different one? What does the item data look like (on the Item Reader addon, enable the "Show item data" option in the configuration window)?
