You run around with 3 HP and use Freeze Traps so you never get hit. You only need to heal when you anticipate that an enemy is actually going to hit you, either because you messed up or RNGsus frowned upon you.
HP stealing is quite valuable for saving mates if you're running out of them...
The icon is determined by the animation type. Saber, Dagger, Sword, Partisan, Slicer, Claw, Twin, Fist, Katana, Swords have the saber icon. Handgun, Rifle, Shot, Mechgun, Launcher have the gun icon. Cane, Rod, Wand, Cards have the cane icon.

That's a bit hyperbolic. The whole point of the Section ID system is to force you to either play with other people or make multiple characters. The previous system invalidated Section IDs to the point where they might as well have been abolished entirely in favour of having a menu to select your...

I've recorded a little global flag data over the years, but more recently I decided to bite the bullet and just document everything. Here's the list of mostly GC stuff:

It doesn't work properly with any costume. You should be able to see the eye from any angle, but you can only see it on the opposite side of the face in GC and BB.
Monster NPCs and script NPCs are completely different. The monster NPCs are just part of the map data and don't consume a slot, but their assets are only loaded in specific areas, just like enemies. Script NPCs are treated like actual players and consume a slot, but you can use them anywhere...
PSO has rigid lists of enemies and objects supported in each area, most likely to fit the game within the Dreamcast's 16MiB RAM. You cannot put NPCs, enemies, or objects in areas where they are not supported, since PSO only loads the assets in supported areas.

Seriously though, have you installed any skins? You could have a corrupt texture archive.
BB does not support Xinput controllers at all. You need a DirectInput driver to use your controller with PSO. I have no idea how you would have gotten it to work before unless it was enabling DirectInput silently without your knowledge.
Sylverant and SCHTServ are the two main servers that support GC. All you do if you have a BBA is set DNS1 to the IP of the server of your choice, and you'll connect there. Both servers require registration before you can play.
